Humberstone Medical Centre welcomes all new eligible patients to the practice.

To see whether you are eligible by location to join the practice, please check the locations we cover in east Leicester.

If you are not sure whether you are eligible for NHS treatment, please contact the surgery by phone or email.

If you are not eligible for NHS treatment then you are welcome to join the practice as a Private Patient and take advantage of our range of Private Services.

How to Register

Registering with Humberstone Medical Centre is straightforward and confidential. Providing you are eligible for NHS treatment and eligible for the practice, you may select one of the following two options:

We have made the form as simple as possible for your ease of completion.

Submitting this form does not constitute or imply automatic registration or acceptance onto the register of patients at Humberstone Medical Centre.

Once you have submitted this form on line, please attend the surgery in the next 5 working days BRINGING WITH YOU:

  • Any medical cards or NHS numbers for each person registering.
  • The Red Book for any children aged under 16.

Proof of address AND identity (please note we will only accept the following):

  • Valid driving licence (both parts)
  • Valid passport plus proof of address
  • Current local authority rent card
  • A utility bill (dated no more than three months ago)
  • Recent bank or building society statements
  • Recent payslip plus proof of address or P45
  • Recent documentation from DWP showing name and address

 you will be asked to complete NHS Ethnicity and Health Promotion forms to complete your registration. Thank you.

WE WILL NOT BE ABLE TO REGISTER YOU IF YOU ARE UNABLE TO PROVIDE PROOF OF ADDRESS AND IDENTITY.
